With the project \"Further Development and Expansion of a Social Space-oriented, Integrative Care Structure for Elderly People and People in Need of Care in Saarbr\u00fccken-Brebach\" in the Model Programme SGB XI \u00a7 45 c para. , Diakonie Saar is pursuing the goal of making low-threshold care and relief services more accessible to people in need of care and in their immediate living environment. The focus is on securing self-determined living in a familiar environment - also in the case of need for care - as well as relieving the burden on caring relatives.<\/p>\n
Da die Bed\u00fcrfnisse der \u00e4lteren Menschen im Quartier als Ma\u00dfgabe gelten, wurde ein sektoren\u00fcbergreifender Ansatz gew\u00e4hlt. Pflegebed\u00fcrftige und von Pflegebed\u00fcrftigkeit bedrohte Menschen sollen Hilfen aus einer Hand erhalten und in die Lage versetzt werden, diese abrufen und finanzieren zu k\u00f6nnen. F\u00fcr diese Herangehensweise (\u201eBrebacher Modell\u201c) wurden in Vorg\u00e4ngerprojekten die Grundlagen geschaffen.<\/p>\n
Der Arbeitsansatz soll nun weiterentwickelt und in einen verl\u00e4sslichen Finanzierungsrahmen gebracht werden.<\/p>\n
Zur Unterst\u00fctzung bei der Umsetzung dieser komplexen Aufgabe in einem benachteiligten Stadtteil sowie zur Sicherung einer hohen fachlichen Qualit\u00e4t und Nachhaltigkeit wird eine wissenschaftliche Begleitung durch das iso-Institut eingesetzt. Die Begleitung umfasst in erster Linie eine Beratung der Projektverantwortlichen im Rahmen von Reflexionsgespr\u00e4chen und Workshops sowie die qualitative Analyse von Nutzerbed\u00fcrfnissen und Optimierungspotentialen.<\/p>\n
