Process Support in the Project "Hospice and Palliative Culture in Catholic Institutions and Services"

The present pilot project aims to further develop the hospice and palliative culture in Catholic Institutions and Services. To this end, cross-sectoral networks will be established in the regions of Saarbrücken and St. Wendel. Within the framework of the project, a jointly developed basic instrument designed to identify hospice and palliative needs of seriously ill and dying people will be implemented. And beside this, the instrument will enable coordinated cooperation with all those involved in the care system. The goal of this project is to establish a common hospice-palliative understanding.

The process supported by the iso-Institute aims to ensure the exchange of experience on the implementation of the Thebasic instrument across facilities and thus to support the participants in the associated coordination processes. At the end of the process, the results will be incorporated into the facility-specific quality management and consequently will be made permanent.
