The focus here lies particularly on digitalisation and virtualisation of the working world as future-oriented structural change requires transferable solutions. Project Duration: September 2020 – August 2023Heart failure (CHF) is the most common single diagnosis for hospital admissions and is one of the leading causes of death. The majority of patients are over 65 years of age, and demographic trends and improved survival rates for cardiovascular disease will increase the importance of heart failure as a burden on the health care system in the future. In addition, there is a very high rate of readmissions (rehospitalisation) of these patients. The reduction of readmissions for inpatient treatment is therefore an important task for relieving the burden on the healthcare system. This is where the European project PerHeart comes in: Through the use of information and communication technology (ICT), the health care of HI patients is to be demonstrably improved and the re-hospitalisation rate of those affected significantly reduced.
